AVID Microchip Change Information Form

If you have moved or obtained a pet that was Microchipped prior to your purchase or adoption you should fill out this form and fax it in the the Hawaiian Humane Society.

Please also fax a copy to us here at the clinic (808) 235-1754 fax, so that we can update your records.

AVID Microchip Registration Form

Haiku Veterinary Clinic recommends each patient to have a Microchip. Implanting a Microchip usually costs the client $45.00. This $45.00 insures that you are the owner of this pet should the pet be lost/stolen or turned into the Hawaii humane Society. This is most important during natural disasters.

If you pet was injured and sent to a veterinary hospital with a microchip you would be contacted to facilitate your patients care. If the humane society picks up your pet and it has a microchip they can return your pet to you. Many times this can be without the need for you to go to the Humane Society and pay the fees for "getting your dog out of jail".

Microchips are the size of a rice grain and are implanted between the shoulders of the patient in a simple procedure though an injection. This is an outpatient procedure.

Attached is the form to register your pet's new microchip with the Hawaiian Humane Society. There is no cost involved in registry. Please fill out this form and bring it with you to your appointment.

C & C Form to Prove Pet is Spayed/Neutered

Attached is the form to show the City & County of Honolulu that your pet has been neutered.

This form is important to have if you are registering your pet with the city for licencing and it has been spayed or neutered.

City & County Pet Licencing Form

Attached is the form required for registering your pet with the City & County of Honolulu. If your pet has been spayed or neutered you will also need to print the form: C & C Form to Prove Pet is Spayed/Neutered. This form will reduce the price of your City & County pet registration.

Military Clients: Clients who live on any of the military bases here in Hawaii are required by the military to have their pets registered with the City & County.
